Missed appointments without 24 hour notification will be charged $50. Missed appointment fees are not reimbursed by most insurance carriers.

All information is confidential unless a release of confidentiality is signed. Law requires breach of confidentiality if there is reason to believe there is child abuse, elder abuse, if client may be dangerous to self or others, and in some court cases. Some insurance carriers require client information. If this is the case, you will be asked to sign a release.

From the intersection of Mt. Herman Road and Scotts Valley Drive, turn onto Scotts Valley Drive towards the Shell Gas Station. At the Bank of America, make a U turn. Turn right onto Jolley Way, a very small road which leads directly into the parking lot for my office building. If you pass Jolley Way, you will need to go back to the intersection of Mt. Herman Road and Scotts Valley Drive and start over again. My office is on the 2nd floor in the back of the building. You may use the back staircase with an unmarked door or use the main lobby.

20 S. Santa Cruz Avenue is downtown Los Gatos across the street from the park and the Post Office. Suite #309 can be difficult to find because some of the suite numbers on my building are not sequential. Suite #309 is above the valet parking lot on the balcony side of the building. I will validate parking if needed. From the San Jose Airport, take 101 South towards Los Angeles. Take I-880 South which becomes Highway 17 South. Take CA-9 exit (Los Gatos/Monte Sereno/Saratoga). Turn left on N. Santa Cruz Ave. until you cross Main Street, at which point N. Santa Cruz Ave. Changes to S. Santa Cruz Ave.  Continue one block, turn right at Broadway and then turn right again into the valet parking lot.
