Missed appointments without 24 hour notification will be charged $50. Missed appointment fees are not reimbursed by most insurance carriers.

All information is confidential unless a release of confidentiality is signed. Law requires breach of confidentiality if there is reason to believe there is child abuse, elder abuse, if client may be dangerous to self or others, and in some court cases. Some insurance carriers require client information. If this is the case, you will be asked to sign a release.

From Skyway Road, take Bruce Road for 2.7 miles.  Bruce Road ends near Enloe Outpatient Treatment Center.  Turn right at the light onto California Park Drive.  Turn into the next drive way on your right. 

See the sign: Lakeside Psychological Services. Stay to the left as you drive through the complex towards the building in the back.  My office is on the second floor.

Please watch for ducks and geese in the roadway while driving through my office complex.  At night be extra careful not the hit the ducks that might be sleeping in the road. 

From the intersection of Clark Road and Elliot Road, continue South on Clark Road and see the Quail Run Business complex on the left.  Drive into the main entrance of Quail Run and park in the back. My office is on the second floor nearest the back staircase. The letter “M” is above the door.
